Disease Management Program

FHN Disease Management Program

You are identified for Disease Management by our Case Management team or at your medical provider’s request. You or a caregiver may also refer yourself to our Disease Management Program by calling the number below.

We want to help our members live normal, healthy lives. We can do this by teaching our members to manage their long-term conditions. Our Care Coordinators will listen to you and help you get what you need to manage your condition. Based on sound clinical practices, these programs are formed to support your Medical Provider’s care and treatment plan. This can be done through:

  • Member outreach
  • Education
  • Monitoring
  • Self-management

Programs now available are:

  • Diabetes
  • Asthma
