Welcome to the FHN Physician Provider Search. To find a physician, use the fields below to help narrow down your search. You can select one or multiple search fields. The more fields you select the more narrowed down the results will appear. If you are unsure of what something means, select the blue help button (?) next to the field. A pop-up will appear with a detailed description of the term. Once you are done entering the fields, select the Search button to receive your results.

Specialty: Specialty is the type of medicine that a provider practices. The provider’s training and education is checked when they apply to join us and once every three years. If a provider completes additional education and training, the information is verified and updated within two weeks when notified of a change. For a full list of specialty terms, click here. Limitation: Specialty can change in between review cycles. We count on providers to tell us when there are changes.
Medical Group Affiliation: Medical group affiliation indicates that the provider is connected with a certain medical group. This is confirmed when they apply to join us and once every three years. Updates are made within two weeks of when notified of a change. Limitations: Medical group affiliation can change in between review cycles. We count on providers to tell us when there are changes.
Name: Our providers self-report their name when they apply. FHN reviews this information every year. Updates are made within two weeks of when we are told of a change. Limitations: Name can change in between review cycles. We count on providers to tell us when there are changes.

Hospital Affiliation: These are the hospitals in which our doctors work that are in our network. This is confirmed when they apply to join us and once every three years. Gender: We take down a provider’s gender when they apply to join us. We check this information once every three years. Updates are made within two weeks of when we are told of a change. Limitation: Gender can change in between review cycles. We count on providers to tell us when there is a change.
Languages: Our providers tell us which languages they and their clinical staff speak when they apply. FHN reviews this information every year. If there is a change, we will update the list within two weeks. Limitations: The languages providers and their clinical staff speak can change in between review cycles. Accepting New Patients: When one of our providers decides they will not be accepting new members they can choose to close their panel. This means that the provider may not be able to see new members. The Illinois Department of Healthcare and Family Services can also close a provider's panel. This could happen if the provider is not eligible to accept new patients. Providers tell us if they are accepting new patients when they apply to join us and once every year. Updates are made within two weeks of when we are told of a change. Limitations: Whether a practitioner is accepting new patients can change in between review cycles. We count on providers to tell us when there are changes.

FHN serves people living in the Chicago area: including Cook, DuPage, Kane, Kankakee, Lake and Will Counties.

Service outside of our Service Area

If you are not in our service area and you need care but it is not an emergency, please call us at 1-888-346-4968 TTY: 711. We will work with you to help you get the care and services you need. When FHN helps to arrange non-emergency services for you outside its service area, we will cover the cost. If you get services outside of our coverage area, we may not pay for those services.
If you are not in our service area and you need emergency care, call 911 or go to the nearest hospital.
