Enrollment Requirements
You may qualify for Family Health Network if…
• You live in COOK COUNTY
• You have an HFS Medical Card
• You have an ALL KIDS Medical Card
• You have a FAMILY CARE Medical Card
• You have a MOMS & BABIES Medical Card
Joining Family Health Network
You can enroll in Family Health Network by choosing to:
• Call FHN Member Services toll free at 1-888-346-4968 (TTY: 1-800-422-1942)
• Call Illinois Health Connect toll free at 1-877-912-1999 (TTY: 1-866-565-8577)
• Enroll online at: https://www.illinoisceb.com/enroll/
Please have your HFS card ready!
After enrollment:
• You can choose a Primary Care Physician (PCP) for each family member
• You can choose an FHN Women’s Healthcare Provider
• You can change your PCP if not satisfied
• You will receive Basic Medical and Extra Benefits
