$10 Target Gift Cards

FHN Members can earn $10 Target Gift Cards for continuously attending pre-natal visits.

When you let FHN know that you are pregnant, you will receive an incentive form in the mail to take to your pre-natal visit. When you attend your visit, have the incentive form completely filled out by your doctor or office staff. After the visit, send the completed form back to FHN to receive the $10 Target Gift Card.

FHN will mail the gift card and a new pre-natal incentive form for your next visit to the address you provide. Make sure you provide the correct mailing address when you complete all forms.

$25 Target Gift Card

FHN Members can earn a $25 Target Gift Card for attending their post-partum visit 21-56 days after birth AND completing the Depression Screening.

After you deliver, you will receive an incentive form in the mail to take to your post-partum visit. When you attend your visit 21-56 days after birth AND complete the Depression Screening, have the incentive form completely filled out by your doctor or office staff. After the visit, send the completed form and Depression Screening back to FHN to receive the $25 Target Gift Card.

FHN will mail the gift card to the address you provide on the form. Make sure you provide the correct mailing address when you complete all forms.

Picture Me Baby Photo Program

FHN Members can receive a Sear’s Portrait Studio Coupon to use for their baby’s first picture through Picture Me Baby.

To receive this incentive, you must deliver in-network, attend your post-partum visit 21-56 days after giving birth AND complete the Depression Screening.

As an extra reward, you can receive a Sear’s Portrait Studio Coupon once a year for that child, from birth until the age of 5 years old, if your child remains an active FHN Member and you keep his/her immunizations up-to-date.

Diaper Program

FHN Case Holders can receive a diaper coupon for a Free Jewel-Osco Baby Basics Jumbo Pack of Diapers for keeping their children’s immunizations up-to-date.

Attached to the coupons are Immunization Requirements that allow members to continue participating in the monthly program. Members can receive monthly diaper coupons for their child until the age of 3 years.

When you receive the diaper coupons, take the coupons to any participating Jewel-Osco store. You will receive a jumbo pack of diapers when you show your FHN ID Card along with the coupon. Always carry your FHN ID Card!

Mammogram Program

FHN Female Members, age 40 and over, can earn a $25 Payless Shoe Source Gift Card once a year by receiving their annual mammogram. FHN sends out a letter once a year to remind women to get their annual mammogram. If it is time for your annual mammogram, call FHN to receive the Mammogram Incentive Form OR click on the word to print.

When you go to your mammogram appointment, have the doctor or office staff complete and sign this incentive form.
