FREE Transportation for medical office visits, pre-natal and post-partum visits, and birthing classes. Members can set up transportation by calling 1-888-346-4968 at least 24 hours in advance during FHN business hours 8am-5pm.

No Co-Payments for FHN members for physician visits, Emergency Room, hospital services and covered prescriptions when they show their FHN medical card at FHN contracted providers.

*Members must carry their FHN ID Card and HFS medical card to insure no co-pays.

Brighter Beginnings Program for pregnant women and babies. Program benefits include pregnancy booklets, case management and free transportation to medical visits and birthing classes. Members can earn $10 TARGET GIFT CARDS for attending pre-natal visits, a $25 TARGET GIFT CARD for attending their post-partum visit AND completing their Depression Screening, and a Diaper Bag for delivering in-network and missing NO MORE than 1 of the Required Pre-Natal Visits.

You must call Member Services to enroll in Brighter Beginnings and inform FHN that you are pregnant. You will receive a welcome package that includes the Brighter Beginning incentive form which you are required to take to your pre-natal visits and have filled out by the doctor or the office staff. Once completed, mail the pre-natal visit incentive form to FHN. When FHN receives the form, the $10 Target Gift Card and the next pre-natal visit incentive form will be mailed to the address you provide. *Each member can receive a gift card for each pre-natal visit they attend.

You must inform FHN when you deliver. If you deliver in-network and have missed NO MORE than 1 of the Required Pre-Natal Visits, you will receive a FHN Diaper Bag.

FHN will send you a post-partum visit and depression screening incentive form. You must attend the post-partum visit within 21-56 days after giving birth AND complete the Depression Screening. The incentive form must be filled out by the doctor or office staff and mailed back to FHN. Once FHN receives the completed incentive form, the $25 Target Gift Card will be mailed to the address you provide.

Free Adult Dental Services for a FREE office visit and dental cleaning every 6 months, limited root canal AND scaling and deep cleaning every 2 years upon approval. Members can call 1-888-346-4968 to find a provider near them or request a List of Dental Providers and services.

$20 Vision Rebate for Adults to be used once a year for frames or contacts purchased from any licensed provider. Members must submit an itemized receipt to receive their $20 rebate.

FHN Adult members call Member Services to receive a rebate form. When the member purchases frames or contacts from any licensed provider, they will receive an optical center itemized receipt. Members will send Family Health Network the itemized receipt and the completed Vision Rebate Form. When FHN receives the completed form and receipt, a $20 rebate will be mailed to the member. *Each member can use this rebate once a year.

Children’s Book Club for children ages 5-16. Children can earn special rewards for reading when they enroll in the book club and send FHN 3 book reports quarterly. They will receive a $10 Target Gift Card, book, book bag, reading certificate and can be featured in the FHN monthly newsletter.

FHN Members must call Members Services to enroll their children into the Book Club Program. We will mail out the Children’s Book Club Form to be completed. Child must complete the book club form, attach 3 book reports and mail it back to FHN. Once FHN receives the form and reports, FHN will mail a book, a certificate, $10 Target Gift Card and a book bag to the address provided.

Free Weight Watchers® Membership and weekly meeting coupons. Members may participate in this program if they meet the age, weight and height requirements. They will receive free weekly coupons to attend meetings when they send FHN their signed weekly weigh-in sheet.

FHN Members are required to contact FHN Member Services to enroll in Weight Watchers. Once enrolled, members are required to attend weekly meetings and mail or fax authorized coupons to FHN in order to receive additional coupons.

Discounted Curves® Membership and waived fee. FHN FEMALE Members age 18 and over can receive a Curves membership discount of $32 a month and will have the joining service fee waived (originally $99) when they enroll with FHN.

This extra benefit can be used at all the Curves locations in Cook County. In addition to adult females, any female FHN Member age 12-17 may work out with an eligible adult member for $17 a month.

If you are interested in joining Curves, please contact FHN Member Services to request Curves membership information and the discount coupon. Members must present the Curves Coupon and FHN ID Card when they go to any of the Cook County Curves locations to sign up for the discounted membership of $32 a month.

*Curves discounted membership requires a 3 month agreement and can be paid by check, money order or credit card. Members can renew their Curves membership every 3 months by visiting their Curves location.

Health Education for FHN Members who would like to receive free health information on topics of interest. Members can request information about pregnancy, pregnancy diet, breastfeeding, asthma, diabetes, smoking cessation and more. Call Member Services to request free health information.

Pregnancy Tests for home use. Members can call FHN and request a free home pregnancy test kit. A FHN case worker will contact the member to confirm the pregnancy test results and enroll them into the Brighter Beginnings Program.

FREE $1 JEWEL-OSCO Coupons to use on any Equaline product (examples: vitamins, supplements and first-aid products). FHN members can call Members Services and request up to 5 Jewel-Osco coupons a month to use at any participating Cook County location.
